Complete Our Online Registration Form
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

The Day of Your Visit
You will need to review our Pre-Operative Surgical Instructions before coming in for your visit. Most importantly, if you are having surgery, you may not have anything to eat or drink for six hours prior to your appointment. You must also plan for a responsible adult to accompany you to the office, remain in the office during the procedure, and drive you home after your appointment.
If your appointment is for a CONSULT ONLY, the eating and driver requirements do not apply.
